Our support planners assist with the paperwork and decision-making needed to help utilize available resources while aligning with personal goals and needs. A community support plan may include paid and unpaid services, as well as formal and informal supports, all within the consumer-directed community support (CDCS) budget.

Moreover, our support planner’s role will include educating you about CDCS and the best possible options, developing a community support plan, finding community-based supports and services, and making purchase and delivery arrangements for such supports, among other things. We also provide technical assistance with recruitment, which includes screening, hiring, training, scheduling, monitoring, and payroll.
